Application Information
Application deadline is January 8, 2010. All materials must be received by that date to ensure that your application is considered. Application materials include:
- Online application completed at http://depts.washington.edu/uwrxres Instructions can be found on that site.
- Letter of intent stating your career goals, interests and the reason you are considering this program. (uploaded with online application)
- Curiculum Vitae (uploaded with online application)
- Official transcripts of all professional pharmacy education (mailed separately)
- Three letters of reference from healthcare professionals who can attest to your practice abilities and aptitudes. These recommendations can be completed by your reference online at http://depts.washington.edu/uwrxres Instructions can be found on that site. Although we prefer the online submission of letters of reference, a PDF form is available that can be filled out and mailed or faxed to us. CLICK HERE FOR THE FORM.
- An on-site interview is required. Applicants selected for an interview will be notified via email by January 22, 2010.
Applicants for the Residency in Health-System Pharmacy Administration/Master of Health Administration have these additional requirements (checklist available):
- One additional letter of recommendation addressing your readiness for leadership from someone in a position of leadership
- Applicant self-assessment form
- GRE Scores (not required for applicants to this residency unless they have a GPA<3.0/4.0)
- An example of a completed project (i.e. DUE, formal drug information write-up, presentation, etc.)

Applicants for PGY2 programs that are selected for an interview will be expected to give a presentation on a topic of their choice. Please contact the specific program director for more information.
